This is an original pattern.

S6499 1964 Dress with high round neckline and front zipper has patch pockets and optional tie belt.  V1 has collar and short set in sleeves.  Sleeveless V2 has collar.  Collarless and sleeveless V2 is top stitched.

Printed pattern is missing the pocket piece.
